Potassium

• 10 mg → 0.1 mmol/L increase, no matter the route
• Options:
  • K-Dur (PO)
    • Cannot be crushed
    • 20-40 mEq, can be provided up to 80 mEq/day
  • KCl liquid solution (PO); for those who cannot tolerate a pill
  • KCl in NS. 40 mEq/L – pt has to tolerate ++ fluid
• Try to keep K > 4 mM for patients with an arrhythmia

Magnesium

• 2 g IV intermittent
• 0.7 mmol/L is the threshold for replacement
• Try to keep Mg > 1 mM for patients with an arrhythmia

Phosphate

• At UHN: phosphate novartis
• Dosing: 500 mg PO BID, 2 tabs, x 3 days

Calcium

• Calcium carbonate 1250 mg PO BID

Sodium

• Free fluid restriction
• Consider dialysis
