Potassium
- 10 mg → 0.1 mmol/L increase, no matter the route
- Options:
- K-Dur (PO)
- Cannot be crushed
- 20-40 mEq, can be provided up to 80 mEq/day
- KCl liquid solution (PO); for those who cannot tolerate a pill
- KCl in NS. 40 mEq/L – pt has to tolerate ++ fluid
- Try to keep K > 4 mM for patients with an arrhythmia
Magnesium
- 2 g IV intermittent
- 0.7 mmol/L is the threshold for replacement
- Try to keep Mg > 1 mM for patients with an arrhythmia
Phosphate
- At UHN: phosphate novartis
- Dosing: 500 mg PO BID, 2 tabs, x 3 days
Calcium
- Calcium carbonate 1250 mg PO BID
Sodium
- Free fluid restriction
- Consider dialysis
